Fantastic and well written letter. I don't think that we are getting accurate information in British Columbia. One problem with the reporting of adverse events in Canada is that only a doctor can do the reporting, which leads me to believe these adverse events are being under reported.

According to the Canadian Adverse Reporting, these are all of those who can/have posted adverse events up to and including death:
Consumer/other non health professional
Nurse
Other health professional
Pharmacist
Physician
Problem is, a lot of these reports are being rejected for numerous and various reasons.
